Physician's Report for Residential Care Facilities — LIC 602A
A Physician's Report and TB test are required by the California Department of Social Services for admission to any Licensed Assisted Living Facility. This form is completed and signed by the potential resident's primary physician to assess current functional capacity, mental condition, and any pertinent health-related considerations.
